PURPOSE:
To provide consistency throughout the University in answering inquiries regarding present or former faculty or staff members.
INQUIRIES ADDRESSED TO THE UNIVERSITY:
Inquiries addressed to the University (rather than to an individual)should be forwarded to the Manager, Information Systems and Records Division, Room 517 James M. Elliott Building, 865-1414. Inquiries regarding student employees must be directed to the area where they work.
INQUIRIES CONCERNING HEALTH CARE CLAIMS AND COVERAGES:
Inquiries concerning an employee's health care claims and coverage, and related matters should be referred to the Manager of Employee Benefits, Room 114 James M. Elliott Building, 865-1473.
INQUIRIES CONCERNING WORKERS' COMPENSATION MATTERS:
Inquiries concerning workers' compensation matters should be referred to the Office of Human Resources/Workers' Compensation, Room 312A James M. Elliott Building,(814) 865-0424.
INQUIRIES CONCERNING RADIATION EXPOSURE RECORDS:
Inquiries concerning an individual's radiation exposure records should be referred to the Health Physics Office, 228 Academic Projects Building.
INFORMATION REQUESTED BY LEGAL MEANS:
Information requested by legal means should be channeled through the Office of Human Resources, Room 505 James M. Elliott Building.
